Multiple entries for one field

Hi,

Ever post a question here and then it completely
disappears? Anyway, I am new to this so please excuse my
ignorance. I need to set up a d/b for survey results and
a lot of the questions allow for more than one answer,
i.e. 'Check all boxes that apply'. How would I go about
setting this up? Ulitmately I would like to design a form
that allows the user to enter data on these questions by
checking the boxes just as shown on the survey paper but I
do not know how to program the table to accept multiple
entries. If anyone can advise me I would greatly
appreciate it. Thanks in advance.
