Multiple records in a query.

I've created a query that keeps giving me duplicate records when run.
I've looked at it so long that I'm obviously overlooking somethin
thats there that isn't suppose to be.

Can anyone tell me what are some of the causes for this? Or what
might look for?

I'm totally confused!!! As you can see in the attachment.
