My add picture icon is gone Where is it in the field chooser?

I am trying to customize a form and added some new on the general tab. Now,
the icon that used to display "add contact picture" is gone. I'm trying to
find out where that field is in the field chooser list, and I can't find it.
Does anyone know how to reinsert?

Also, my form doesn't show up when I click add contact. I've designed and
published my form. How do I get it to be the forms template so everytime I
click on a new contact, the appropriate form is displayed? Some, under
Actions, I see my form name at the bottom of the list, and when I click on
it, the proper form appears. Anyway to convert older contacts to view the
same form?

That's one of several bits of functionality that you lose if you customize the General page on the contact form.
