My check box shows on report but not when printed?

I created a survey table where users check the box if an itam is completed.
When I go to print preview on the form I see the check box, but when I
physically print it, it isn't on the paper. Whether checked or not, it
doesn't show up. So then I created a report from a query pulling the data.
Again, see the check box on the print preview of the report but it is not
showing when I hit print. Help????
