My header/footer "Insert Autotext" drop-down is blank

You don't need AutoText for this. You can use Insert | Field to insert
either the "Filename" field or the "Title" field, depending on what you mean
by "Document Name." But if you have no AutoText entries, it's most likely
because you've saved a document as Normal.dot. For help in creating a fresh
copy of Normal.dot, see
