mysterious missing toolbars

I have lost the top toolbar (main menus) - file;edit;view..etc etc
Therefore cannot get to Tools menu to reset options to the default settings.
How can I restore this toolbar?

I've seen it happen with I.Explorer also...several colleagues at work have
suffered same probs.
HELP HELP HELP!
What can i do?
