Name and address sideways in margin for pleadiing paper.

I am required by court rule to put my name, address and contact info sideways
in the left margin of numbered pleading paper. I have a sample, but can't
change my old name and address.

View | Header and Footer to access the (presumed) text box that contains the
data.
