I've never seen a template for that. Having gone thru that process myself
not too long ago, some of the companies want to see you in person, some will
do it on a phone call, some want to see a copy of your marriage certificate.
The company where I worked at the time wouldn't change my name until I had a
Social Security card with my new last name. There is no one size fits all
solution.
