Need a solution

I am a school principal and need a form that teachers can fill out
disciplinary information about a child and email/send to me and I can add
data about student consequeces and send it back to teacher and a copy to a
student file. What type of application would I use to create such a form?
InfoPath, Word, Adobe Acrobat? Once I know what application to use I will be
able to design a pretty simple form to use.
One note...eventually, but not now, I would like to be able to track the
student disciplinary data in a database.

If you want to track the information from the form in a database then if you
have InfoPath that would be the best tool for the job.

It has the ability to bind the data entry fields to a database and the
ability to easily extract the data into an Excel workbook if you want to
take that route. Plus it has the added bonus of ease in data validation (if
that's something you need) and you can add a button to the form to send it
by email (no coding necessary). You can pre-configure the recipient,
subject, etc. All they have to do is fill out the form and click the button.
You can't get any easier than that!

Depending on the version of Office and InfoPath you are using, InfoPath 2007
also has the ability to work in conjunction with Outlook 2007. Upon receipt
of the emailed form, an InfoPath folder will be created in Outlook and the
forms will automatically routed the folder. Unlike other folders, the
InfoPath folder contains additional commands for working with the data, such
as the ability to view the data from all of the forms in the folder in Excel
with a click of a button.

Thanks Beth,

Do all the members of my faculty have to have infopath for this to work? Or
can I be the only one with infopath? Does this need to be placed on a server
or can it operate from individual pc's in a network?

This depends on the version of InfoPath and additional software. If using
InfoPath 2003 each person needs a copy of InfoPath. If using InfoPath 2007
with Office SharePoint 2007 (with Office Forms Services) then the InfoPath
form can be hosted on SharePoint and each user doesn't need their own copy.
