Need direction on creating FORM

Hello !!
i am a teacher given the task of seekin out the possibilties of
creating a "report card template" or FORM that our teachers could use
to enter data and have print out results. The data would consist of

student name
Teacher
12 Subjects areas, with 4 reporing period
Comments boxes for 4 reporting periods
Attendance: monthly averages
behavioral/ attitudes


all i want to know is. am i using the right app - excel or word. or is
there another application i should use.??????????

Note: i am trying to keep this simple and very straight forward for
our users. computer skill levels are low with our staff including
myself. =-)

Before I created the form, I'd look to see if there's anything I could steal.

The first place I'd look (but I didn't!) would be the MS site:

Download the Teaching and Learning Resources

If I didn't find something useful there, I'd try google:
http://google.com/search?as_q=teaching+"report+cards"+templates+excel
I got 366 hits when I search for:
teaching "report cards" templates excel

Then if I were really ready to bite the bullet and do something myself
(yechhh!), I'd look at these two links that explain how to use mailmerge (keep
the data in excel, but build the form in MSWord):

Well thanks very much

I spent a hours loooking at looking through the tips. and still am
looking through them..

I am concidering creating my own.. but if i can find one FREE! then so
be it!! Minutes are precious for me. Hours are no longer mine so
finding free stuff is always a wiser choice for me....

thanks very much all yall for youf input and advise.

NOTE: do you know of any of these computer geeks that i
could ask? We are fresh OUT around here. ;-)

My last comment


I do apologize for my poor grammer. I usally dont spend enough time
editing my entries for anything I write on the net ( a weakness yes).
And you do deserve better from a teacher. =-(
Thanks again for your help. I will write soon because the mailmerge
thing sound really fuzzy.

True fact: I was offered an grade 8 english class this year and i
happily declined. lol

I wouldn't worry about the grammar--unless the CQC (Canadian Quality Council) is
up and monitoring the posts.
