Need Form Help

Hello,

My pre-emptive apologies if my post is inappropriately placed, or too
pedestrian. While I'm quite computer savvy (hardware technician, self
employed), this sort of thing is not my forte, and I'm not too proud to ask
for help.

I'm not even sure if what I want to do is possible, or if I can only do this
for as many of the form's recipients as possible...I'm sure that this is
laughably
simple for the regular attendees to this forum, and snicker if you will.

I need to create a form, using Microsoft Word, if possible. If necessary, I
can spring for Adobe Writer, or better yet, deskPDF, since it's so much
cheaper. It's a very basic form, prompting for very basic info such as:
Name, Address, Phone Number, etc.

Those field names should be write-protected, so that the people who fill out
the forms cannot alter them. Of course, after the field name, the next
field should be read/write, so that the person can fill in the appropriate
info.

When I attempted to follow the MS Word 2003 "Help" instructions, I couldn't
figure out how to set up the read/write field, which was underlined (it
doesn't have to be) so that the completed read/write field "pushes" the
following text and fields, to the right, and ultimately to the next line.

Am I on the right track? Am I making sense? Am I just not looking closely
enough at the Help instructions? Is there any way to set this up so that it
can be opened with Internet Explorer, so if a recipient does not have Word
can do it on a Web page?
