need help in setting up a problem log email template

i need to be able to set up an email template for work colleagues to log
problems they have. basically i need to set up a template that people fill in
eg.


helpdesk template

Sender: i.e. your full name, your department, your phone extension number


Summary (e.g A very brief synopsis of the issue):


Description:
(e.g. a full description of the issue you are having, dates/times, are there
any error messages?, is it a web booking? Which web site was it booked on? Is
it a call centre booking? Is it a BYO booking? Is it a Thomas cook booking?
Flight only? Flight Module issue? Miscellaneous module Issue? Incorrect Fare
showing? Payment Issue?)
