I know Microsoft suggests running 2 instances of PS for when you want to open
PS to the internet. What are the downsides of just using one instance for
internal employees and using the same instance to open up to the extranet for
external customers. Is it something that you would not recommend?

I think the suggestion is a second front end, not a second instance. Maybe
we should call it a second instance of a front end? Why do they suggest
this? SECURITY. The reason for a second front end is to locate it in the DMZ
and to support plain auth over SSL across the Internet. Normally, adding SSL
encryption to an internal system simply degrades the performance without
adding any value.
