Need Simple phone directory Example

Does anyone have simple phone directory example of a form
driven Access database program?

I have looked all over the web and read books without
finding what I would like to do.

I would like a simple swtichboard that allows me to popup
a form to search, add, delete, or modify a phone
directory entry. I do not want to have that navigation
bar and prefer having more control over the application.

I have done this type of application in Cold Fusion using
an Access database backend many times but cannot figure
out how to do it using Access as the front end.

Is this even possible? A simple example posted or sent
to me would be greatly appreciated.
