Need simple template for class reunion bio

I am chairing a 50th reunion and I want to send out a Word document that will
constrain people to put their names (in a certain order) in the same place,
addresses, phone numbers, e-mail addresses and then and allow for the rest to
be open NTE one pate. I can't find this anywhere.

Thanks. Also, I am new at this and need to be told how to get back to my
