Need template for a 6 1/4 x 4 5/8 half fold card

I have some non-Avery half fold card stock that is 6 1/4 x 4 5/8 (folded
size). I am looking for this size template to download and am having a hard
time finding one. I believe that this size is also referred to as "A6", but
am not sure about that either. I have also tried to create a template myself,
but no luck with that.

The web site I ordered these from has no template to use - I didn't think it
would be this hard to find one! Any help would be appreciated.
