Need to create a template in Outlook, using the forms toolbar.

I'm trying to create a template in Outlook, and need it to be setup as a
form, with check boxes and text fields. If I'm using Word to edit my mail
messages, it lets me create the forms template, but doesn't allow me to save
it as an Outlook Template.
