Need to print labels for students activity grades

How can I create labels for students activity grades. These are the fields I
have: Student Name; ClassName; Grade; Conduct Code.

I would like the label to look like this:

Joe Black

Class Name Grade Conduct
Art A S
P.E. A S
Music B S

When I use the wizard label option it prints a label for each class. I only
want one label per student with all the classes to print under the students
name. Please let me know if you need more infomation.

You might be able to create a main report (label type) based on only the
student. Make a small subreportof the class, grade and conduct.
