new actions shown in menu but not in toolbar

I add some custom actions to a form dynamically in the Item_Read (tried
also Item_Open). As a result, when I open the form, the actions are
shown in the Actions menu but not in the actions toolbar (Shouldn't
they even appear in the explorer's context menu?) How can I accomplish
this?
