New Contact Crashes Outlook

I am running OL2002 on Windows 2000. Both are patched to the fullest
extent. OL is running in Corporate Workgroup mode. Whenever I try to add a
new contact, Outlook encounters a problem and needs to close. I send the
error report to MS each time, but I have been unable to figure out the
problem.

Any advice would be appreciated.
