I'm trying to use the "New Letter to Contact" functionality in
Microsoft Outlook. I click on the "New Letter to Contact" from the
actions menu and begin the wizard. I select a templete (from the page
design drop-down) and make sure that "Include header and footer with
page design" is checked.
The problem is, the only thing that makes it to the new document is the
mailing address of the contact in question. All the other images and
text from the templetes are gone. It doesn't matter what templete I
choose. There is a Microsoft Knowledgebase article on this, but it
doesn't seem to help at all as I've done what it has suggested to no
avail. The article in question is:

This same problem exists when I go directly into Word and try to use
the "Letter Wizard". I have tried this on a Windows 2000 machine with
Office 2000 and a Windows XP machine running Office 2003. Both exhibit
the same problem.
