New new lines in a Table (Form) HELP!!!

I want to create a table in a form.

The table will contain a few Form Fields.

The problem is, that I want the table to create a new lines using thes
Form Fields. Right now, when the file is saved as a form, no ne
lines can be created in the table!

Thanks for your help. Here's an example of what I'm talking about.
Obviously, I want the Author, Version and Comment Form Feilds to 'grow
in the form as needed
