In Word 2003, after installing Service Pack 2, Underline Style/Color button
is availalble in Formatting Toolbar. However, after Reset Menu and Toolbar
usage (Customize... Options), this button changed back to the normal
Underline function and there is no way to get it back.

I think you must have customized your toolbar yourself. I have Word 2003 SP2
and do not have an Underline Color button. But you can add it back to your
toolbar as follows:

1. Open Tools | Customize.

2. On the Commands tab, select the All Commands category.

3. Scroll down to Underline Color.

4. Select it and drag it to your toolbar.
