Not all PWA updates appearing in plan

I have users who have entered actual hours in PWA, I applied them, and a few
days later, when reviewing the plan, the hours for a specific day are not
populating. The PWA shows them as being updated, but they are not in the
plan.

1) Why is this happening?
2) Can I apply the actuals? Even though I already accepted them and applied
them, but they are not in the plan.

After accepting the change and choosing update, do you then save and
republish new and changed assignments on the Project?
