If it is a virus, how would Outlook know?

Because it can walk up the stack, find the process id of the program
touching it, lookup its EXE, display that to the user.... or even better
lookup the EXE in the Start Menu and display it's friendly name if it has
one (if it doesn't, that might be a sign that it is a virus). It is
possible.

If it is a "real" virus (rare! i.e. embedded as part of another normal
every-day program), then that's what A/V programs are for. Besides, if it is
a real virus, he's got worse problems.
