Novice and Need Help

About 3 month ago I took on the challenge to create a nonproductivity form
for my staff for them to justify not meeting standards. I used a templete
already given...Absence request. I tried to modify it to meet our need. It
took me a month to get a basic design but then I was unable to retrieve it
from my office email. Since, I've learned how to change my settings. However,
I'm at the beginning again and have truly gotten frustrated. Now I can get
the totals to calculate. I've read the posting but I am truly a novice and
about to give up. Can someone please help me?

The way you've stated your problem, it is not clear what your current
challenge is. Could you please rephrase your question? Or clearly state what
isn't currently working? And which posting did you read?
