Numbering problem in "table of tables"

In word 2007, I created captions throughout my document using the "insert
caption" function. The numbering within the text is fine, but when I created
the table of tables (using the "insert table of figures" option), it skips
many of the tables. This is not a problem with the caption style - they are
all the same. I'm wondering if it's a problem related to the fact that I save
all of my documents in word 2003 format so that others can read them. I tried
re-saving in .docx format and recreating some of the captions, but it still
ignored them. What am I doing wrong?

When you create captions via the Caption dialog box, you choose a particular
label (Table for tables, Figure for figures, and so on). To keep track of
the numbering, Word inserts a SEQ field for each caption. Each caption label
corresponds to a unique SEQ field identifier. For example, table captions
look something like this (if you press Alt+F9 to show field codes): { SEQ
Table }. You may want to display field codes and verify that your table
captions all use the correct identifier. Then try updating the table of
tables.

On the other hand, if you created the table based on styles, make sure the
correct style has been applied to each of your table captions.
