Obtaining the average of option button entries

I've been trying to recreate the Option Button Rating section of the
Applicant Rating Sample Form. However, after carefully recreating all the
ceel in the table, I can not get the average to work. I've reviewed all the
individual setup for each cell and the averae text box. I can not find any
formula Field or Group properties or Table Properties.
