office 2007 GetStarted tab doesn't display

On several systems after I install the Office 2007 Getting started tabs, e.g.
Word2007GetStartedTabSetup.msi, the tabs do not display.

I have installed these successfully numerous times, but they now do not
work. What could be causing this?

To provide a bit more info, this is a COM add-in. When I look at COM add-ins,
it is not checked and Load Behaviour says a Runtime error occurs.
