Hi Howard,
The icon in the Toolbar are described in the registry key:
HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Internet Explorer\Extensions
There has some ClassID under it. These ClassID are described in
HKEY_CLASSES_ROOT\CLSID.
You can refer to the standard to create the shortcut in the Toolbar.
