OL2003,Ex2k3, Custom form and C#

All,

I would like to create a custom form that stores company credit information.
I am using OL2003 that connects to our Exchange server 2003 and want the
facility of loging onto any OL2003 client and downloading the form provided
the user has the correct permissions. I also would like to have this custom
form appear in OWA (Outlook Web Access) that is hosted on the Exchange 2003
so it can be accessed over the web. The choice of development tools will be
C# & ASP.NET - is this all possible and if so can someone point me in the
right direction?
