OL2K custom form lock form fields when checkbox true

I have a custom form pretty much built for the journal section of
Outlook 2000. I've mastered, with Google Groups help, things like date
stamp and other helpful things. The last thing i need help with is I
have a checkbox with the field called completed or something, and when
a user ticks this checkbox, i want all form fields to lock and not be
editable. Can anyone help?

The event that fires will depend on whether that checkbox is bound to an
