OLE error

When I try to insert a jpeg into my form I receive the
following error.
Unknown file format.
Then this next box appears.
Microsoft Photo Editor:
There is a problem loading the OLE object. The object you
tried to create or edit is not a valid OLE object. Re-
create the object and then embed or link it again.

any ideas?
