one form two list boxes - values dependent + dynamic

I have two list boxes one is A list of awards and the other is the year
that awards were received. What I'm trying to do is have the use
select one or many awards and only show the year(s) that that award wa
received.

Once the user has made the selection then he/she clicks on a repor
button that displays the Volunteer Name(s), date of award received, an
any comments in accordance with the list box selections.
