Open contacts in Microsoft Office Outlook 2003

When I open contacts in Microsoft Office Outlook 2003 I get a window along
with an audio warning that says.

Microsoft Office Outlook

The custom form could not be opened. Outlook will use an outlook form
instead.

Only then does the specified contacts information show.

My operating system is XP Pro.

Did you at one time use a custom contact form? Under the properties for
this contact folder, what do you show as the default under the dropdown
"when posting to this folder..."?
