Open Form

I have created a customized form (size 5.5/4.5) with text
form fields to be filled it. The staff wants to be able
to open this form with a keystroke or two (or have it on
a toolbar)or perhaps an AutoText entry. It is saved as a
template. Is this possible?

Any help or guidance you can give me will be appreciated.

The code to create a new document from a template is

Documents.Add "Drive:\Path\TemplateName"

However, if you record a macro as you create a new document from the
template, it will create the code for you.

That's the easy part, maybe so easy that you should just get your users to
do that themselves. Otherwise,
