Try selecting "Normal Word Document", then save and close the document.

Typically, you see the "SQL" dialog box whenever you open a document
that is attached to a mail merge data source.

At a certain point, Word was changed so that this sometimes happened
even when the document was /not/ attached to a data source.

By selecting "Normal Word Document" you /should/ remind Word that the
document is not actually attached to a data source.
