operation failed error when sending custom form

When testing a custom form. I am receiving the
error "Operation failed" after filling in the form and
clicking on the "send" button to transmit the form via
e-mail. I have reviewed microsoft postings on this and
it is not very helpful, plus. I have checked all that
they suggest and all is ok. This error is not very
descriptive. Any help or thought would be appreciated.
