Operation Failed when saving contact

When I try and save a contact after inputting the phone
number I receive an "Operation Failed" message with an OK
button to click on. When I click to "OK" button, the
contact actually saves correctly but what can I do to get
rid of this "Operation Failed" message everytime I save a
contact? I've tried "Detect and Repair" twice and that
did not fix anything.
