Operation Failed

I had a user set up with Outlook 2003 not in cached mode
when she tried to send a message to 150 receiptients, she got an error message
stating 'Operation Failed'

So we set up another profile with Uncached Mode but it failed again

Then we created a cached profile and we went to send the message to all the
receiptients and it worked correctly.

I want to figure out what the issue could be? Any suggestions?
