Options on Forms

I have created a template for an office request form with
check boxes and option radio)buttons. When I use it as a
template or when I open it as a document on my computer,
the buttons all work. When I put it on a shared drive as
a document or a template, the check boxes work but the
option buttons do not. Does this have to do with the
shared drive and is there a way to make the option buttons
work on the shared drive? Any help will be greatly
appreciated!
