Orphaned Header

I have a report that I keep getting an orphaned header. The header is the
third group in "Sorting and Grouping". It's set to with first detail but
it's still being orphaned. Are there any circunstnces where this doesn't
work. I have Repeat header on also for this header. Any direction
appreciated.
