Outline items don't show up on TOC

I created outline title on my Word document and tried to insert an auto
Table of Contents using the feature...however, not all the outline
items show up on the TOC. I checked the levels of these bullets, they
are all fine.

The structure i have is like this

Portfolio 1
-Detail 1
-Detail 2
Portfolio 2
-Detail 1
-Detail 2

For some reasons, some portfolio are showing all the outline correctly,
some are not...this is really driving me crazy...i have been working on
that for my whole day

Please help!
