Outline numbering all messed up

Somehow, the outline numbering of my document has gotten completely messed
up.

I had it working at one point, but now it isn't, and Word refuses to
correct it.

1.1
1.2
1.3
2
1.4
1.5
1.1.1
1.1.2
1.1.3
1.6
1.1.4
1.1.5
3
4
1.8
1.9

etc., etc.

The document was originally written in Word 97 and is now being edited in
Word 2003.

Does any one know what's going on here?

It looks like your numbering is really messed up! So when you get to the
Customize Outline Numbered List box, for each level, delete everything
in the Number Format box and start again by putting in the numbers for
the previous level(s), the full stop and then this level's numbers.
