outline template for a slimline cd jewel case label

I am trying to do some labels for the slimline cd jewel cases as in the cd
singles cases. Does anyone know where I can get a blank outline/template?

Thanks but as I can see they are only for normal cd cases and not the
slimline "single" cases.
