Outlines - Preventing breaks in sequence

I have a very lengthy manual in which I utlized a very basic outlining system

For some reason the document keeps loosing the sequential format - it will change a section that I'm not even working on? I've tried to reformat by selecting Bullets & Numbering, Customize and changing the number or letter to what is 'should' be

Can someone help
