Outlook 2000 custom form checkbox issue

I've created a custom form in Outlook 2000 and it works
well except for one small issue. When the recipient
replies or forwards the form, the checkboxes are
cleared. I'm sure there's a way to prevent this, but I
have not been able to find it.

The text fields work just fine and don't have this
problem at all. Have I missed a step in the checkbox
setup process?

Any help is greatly apprciated.

Are the check boxes bound to Outlook properties? If not, Outlook has no idea where to store the data represented by the check box state. You can set the bound property on the Value tab of the control.
