Outlook 2002: Custom contact form breaks "Email/Display As" link

I developed a slightly modified Contact form by adding a few
additional standard fields to the upper right side of the General
tab. I used the standard form, made the slight changes and then
saved and published the custom form.

The General tab (standard form) allows for 3 email addresses.
Underneath the email address, there is a corresponding field
labeled "Display As". When using the dropdown arrow to select email
address 2 or 3, the corresponding "Display As" field becomes
available for view/edit.

Although these fields were not touched on the custom form,
the "Display As" field no corresponds to the email address, except
for email address 1. In other words, selecting email address 2 or 3
has no impact on what shows in the Display As field anymore.

Any ideas as to why this is happening or how to fix it would be
greatly appreciated.
