Outlook 2003 and Active Sync 4.5.0 (Build 5096) & running XP Pro

In the last month, I have reformatted my PC and all was just great until I
installed Active Sync 4.5.0 (Build 5096) yesterday. Now, after I sync my
Moto Q Smartphone, Outlook is having 'strokes' when I open up the Calendar
(or Task, Contacts) telling me it doesn't recognize the 'custom form' and can
replace it with another. Once I do that, all is ok.
Problem continuing to...I was using Outlook.Maps.live to plot out
destinations for Appointments but the mapping/Location Tab shows
outlook.maps.live is not installed anymore.
I have uninstalled and re-installed outlook.maps.live several times now, in
varying ways and means with re-booting, etc...but still show that mapping is
not downloaded, but I can see it is...
Any help out there?

Try clearing the forms cache.

Brian,
I ran the script to clear the forms cache, un-installed Live Search for
Maps, rebooted, re-installed Live Search and the 'Location' Tab reappeared
but still shows that Live Search needs to be installed.
Any other thoughts on it?
I can use Live Search on the internet but that is long and cumbersome
compared to using the Locations Tab. I was recently told by my Dr that I
have something like Alzheimer's and I'm trying to 'simplify' my life as mush
as I can (while I can), so any other possible ideas would be greatly
appreciated.
