Outlook 2003 and contact form

I have a special contact form
opening of contacts using this form in Outlook 2003 is a
problem. Everytime Outlook is loading the form (not only
for the first time) and every now and then (about twice
in 10 trials) Outlook XP is'nt responding - I have to
close it and reopen Outlook XP
I have tried to change the form, save it and reinstalled
the form but nothing changed
Form was generated as derivate from the original contact
form in Outlook XP and works properly in Outlook XP

Outlook XP running on Windows XP SP1 and all actual
Patches
Contacts on an Exchange-Server 2000 SP2

any suggestion?

Office XP or Office 2003? There are known problems with custom forms in
Office XP/Outlook 2002. There is a fix in one of the service packs. Have
you installed any service packs for Office XP if this is Outlook 2002?

my previous version was Office XP professional - there
the form is ok
now I have installed Office 2003 professional and I have
the problem with hang of Outlook opening contacts with
special forms
